An unconventional overdose
A 60-year-old woman, Margaret, is brought into the ED with altered consciousness.
Margaret is a chronic alcohol user with alcoholic liver disease. Her medications include thiamine 100mg daily and baclofen 20mg twice daily.
Baclofen was prescribed by the liver clinic 10 months earlier for alcohol withdrawal.
Since then, she has had numerous similar presentations with central nervous system and respiratory depression, followed by agitation and aggression upon spontaneous awakening.
She was found acutely confused
