A stray bullet

When an elderly man presented to an ED with gross haematuria, doctors were shocked to discover the unlikely trigger.

The 85-year-old dementia patient had a mildly distended abdomen in the suprapubic region.

Further examination also revealed a hard mass on the ventral side of the penis, as well as the presence of septicaemia.

CT of the abdomen and pelvis found that the mass was, in fact, a 6cm-long bullet lodged in the penile urethra (see A and below).
