Better teamwork could prevent DAA errors

Initiatives aimed at encouraging pharmacists, nurses, prescribers and residential aged care facility staff to work together more effectively could help reduce dose administration aid (DAA) errors.

An Australian study found that improved and more streamlined communication and collaboration between members of the regular and occasional health care team could ensure that the prescriber’s intentions were clear and they were accurately reflected in the medicines repacked into DAAs.

Pharmacists and nurses felt that suboptimal prescriber communication and IT problems
