CDM items tied to 'medical home' under RACGP plan

Patients will have to register with GP clinics to access chronic disease management plans on Medicare, under a radical funding plan drawn up by the RACGP.

Released on Monday at the college's annual conference in Melbourne, Vision for General Practice and a Sustainable Healthcare System attempts to push the so-called patient-centred 'medical home' to the centre of the health debate.
It says a system of voluntary patient enrolment could be rolled out over a period of 3-5 years.
