Errors in 1 in 5 DAAs add weight to Board warning

More than one in five dose administration aid packs sent from Australian pharmacies to residential aged care facilities may be inaccurate.

Researchers who audited 2389 DAAs from 39 pharmacies found there were 770 incidents involving 502 of the packs – a 21% incident rate – after they had undergone an intervention.

The study, results of which were presented at the International Social Pharmacy Workshop in Boston, USA, saw 45 participating pharmacies, as well as aged care facilities,
