GPs implement their own 'healthcare home' models

Six practices in western Sydney are honing a medical home model in the hope that it will eventually go national.

Backed by the primary health network WentWest, the practices are road-testing models of team-based care for patients with chronic disease that they believe can help shape the future Federal Government-funded Health Care Home model.

Two of the GPs leading the charge are practice owners Dr Walid Jammal (pictured), of Hills Family General Practice, and Dr Kean-Seng Lim, from Mt Druitt Medical Centre, who hope to be among the 200 practices to trial health care
