Health care homes model set up to fail

A surreptitious dump of new information about the government’s health care homes trial has doctors convinced it’ll be little more than a cost-cutting exercise.
The health department quietly published the first detailed blueprint of the trial on its website late on Friday afternoon, outlining a three-tiered system of bundled payments for the care of the estimated 20% of Australians with two or more chronic conditions.
