Incorrect DAA packing linked to deaths

The Pharmacy Board of Australia is warning pharmacists to be more vigilant when handling drugs with a narrow therapeutic index after incorrect dose administration aid packaging and checking of methotrexate has been linked to deaths.

In its latest monthly communiqué, the Board said its Notifications Committee had received in recent months “notifications of several medication incidents involving the drug methotrexate resulting in the deaths of the patients”.
