Introduce geographic restrictions on provider numbers

Many of the problems of Medicare would be solved by:

a) Gradual introduction of geographic restrictions on provider numbers. This works for taxi plates, crayfishing licences and pharmacy. It would fix the maldistribution issue and bring a goodwill value back to the business of GP.

b) Registration of patients with chronic disease with a specific practice. The practice then gets some additional capitation payments for managing that person’s illness. It would also allow more efficient use of practice nurses and free up GPs for more demanding clinical
