Introduce geographic restrictions on provider numbers
Many of the problems of Medicare would be solved by:
a) Gradual introduction of geographic restrictions on provider numbers. This works for taxi plates, crayfishing licences and pharmacy. It would fix the maldistribution issue and bring a goodwill value back to the business of GP.
b) Registration of patients with chronic disease with a specific practice. The practice then gets some additional capitation payments for managing that person’s illness. It would also allow more efficient use of practice nurses and free up GPs
