Diabetes medication errors common in GP referral letters

More than 80% of GP referral letters for patients with type 2 diabetes contain errors in medication details, especially for insulin, a study from NSW has found.

A review of letters for 300 patients referred to a tertiary outpatient diabetes care centre found an average of two errors per letter, with the most common discrepancy being omission of diabetes medications being used by the patient.
