Half of device faults, failures not investigated
More than 6800 incidents were reported to the regulator over the decade to late 2011, with 47.5% of these not investigated and a further 25% investigated but with no further action.
Incident reports increased in recent years when the proportion not actively investigated also climbed, the analysis showed.
