Home-based care shows long term benefits for CHF patients

Patients with chronic heart failure who receive home-based care interventions live longer and have fewer re-hospitalisations than patients receiving  hospital outpatient follow up, Australian research shows.

A four-year follow up of 274 participants with CHF who took part in the WHICH study found that that home-based care was associated with a 38% lower all-cause mortality (36% vs 52%) compared to CHF
