Australia was not alone in seeing that general practice needed an institution to support it. Similar meso-level organisations were created in the UK, Canada and the US.

Over time, and with frightening regularity, divisions amalgamated and became networks, reconfigured and refocused and became Medicare Locals (MLs), and now are to become Primary Health Networks (PHN).

One of the problems of regular change has been the difficulty in assessing the true impacts of these organisations
