What you need to know about the government's primary care overhaul

THE Turnbull government is pledging to overhaul chronic disease care in what it describes as one of the biggest health system reforms since the introduction of Medicare 30 years ago.

The grand "Healthier Medicare" plan, unveiled on Thursday, is light on detail, but it promises a trial of "health care homes" in 200 general practices.

The aim is for the homes to co-ordinate medical, allied health and out-of-hospital services as part of a patient's tailored care plan, says Health Minister Sussan Ley.
