Dermatosis neglecta in the infirm
It is called dermatosis neglecta. You can see the adherent waxy, brown scales on the ankle and dorsal toes, coupled with the long toe nails.
When people can’t bend down to towel dry their skin or can’t be bothered to wash, skin scales do not separate off as they should and build up. The same thing happens at the back of the neck and behind the ears and in the ear canals.
Sometimes this condition is just localised to a painful area of the skin where washing causes too much discomfort. Often washing with a soapless wash or using some 5% glycolic
