Faulty ICD leads to patient death

A NSW deputy coroner has recommended mandatory reporting of implanted cardioverter-defibrillator failures by doctors after finding that an undetected short-circuited lead caused the death of a 22-year-old patient in 2011.

Kerryn Foale suffered a fatal ventricular arrhythmia which her implanted ICD failed to convert to a normal rhythm, despite delivering a high-voltage shock.
