Indigenous care plans rising

The Australian Institute of Health and Welfare (AIHW) report found the proportion of patients with type 2 diabetes who had a chronic disease management plan rose from 24.8% to 31.6%, and for patients with coronary heart disease from 22.9% to 33.4%, between 2008 and 2011. 

“This is an indication that more people with chronic diseases are being well managed through primary care services,” said AIHW spokesperson Dr Fadwa Al-Yaman (PhD).
