Lines on nails: Part 2

It is a new pigmented lesion under the thumbnail of an 83-year-old male. No other nails are involved. A new benign naevus is not expected to be arising at this age. The dermatoscope image shows lines of varying width and colour but it is subtle because this is an early lesion and should be all in situ. The patient needs to have the posterior nail fold resected, a 6mm punch to carefully remove the nail plate overlying the nail matrix, and then a biopsy of the exposed nail matrix where the melanoma will be situated.
