A sorry sensation
This lesion occurred without the patient being aware of any injury. Testing sensation showed she had diabetic peripheral neuropathy.
This was full thickness skin necrosis with some pus and a swollen toe. Management involves culturing the pus and then giving antibiotics for three weeks.
Avoid trying to debride the skin as this is likely to worsen the local damage. Peripheral circulation should be assessed with ultrasound.
