Treating to target safe for gout

Fears about treating gout with high doses of allopurinol should be allayed by a study showing gradual up-titration of the medicine is safe, New Zealand rheumatologists say.

More than 180 patients on a creatinine clearance-based dose of allopurinol (or higher) were randomised either to remain on that dose or to have their gradual dose-escalation in a bid to get serum urate below 6 mg/dL, the clinicians from Christchurch Hospital reported.

At the end of the unblinded one year study, 69% of the dose escalation group had their serum urate in the target range of <6mg/dL
