Terms and Conditions – Game of Chance

Information on how to enter forms part of the terms and conditions of entry. Participation in the promotion is deemed acceptance of these terms and conditions.
All references to time in this document are a reference to the local time in Sydney, New South Wales, Australia, on the date stated. The promotion commences at 12:01am on 19 November 2018 and closes at 11.59pm on 9th December 2018.
Entry is open only to practising Australian general practitioners with a valid AHPRA number, who are registered with an Australian Doctor Group publication (Eligible Entrants). Australian Doctor Group reserves the right to request proof of identity, registration and current place of work. Australian Doctor Group employees (and their families) and contributors are not eligible to enter.
Eligible Entrants may enter the competition by completing a 12-question survey via https://www.optimalworkshop.com, which will be accessible via emails distributed to Eligible Entrants.
Upon completing the survey, Eligible Entrants will be directed to a landing page, whereupon they will be invited to enter their details, including their name and email address. Entry of said details constitutes entry to the competition. Five winners will be selected at random at the conclusion of the competition period. Only one entry per Eligible Entrant will be accepted.
The Promoter is not responsible for any lost, late or misdirected entries. The time of entry will be deemed to be the time the entry is received by the Promoter.
The prizes constitute five $100 Westfield EFTPOS gift cards.
There will be five prize draws, one for each prize, to take place on Monday 10th December at 3pm at Level 2, 26-32 Pyrmont Bridge Road, Pyrmont NSW 2009.
Westfield EFTPOS gift cards are subject to the terms and conditions imposed by the Westfield EFTPOS gift cards provider. Once awarded, the Promoter is not liable for any voucher that has been lost, stolen, forged, damaged or tampered with in any way.
All winners will be notified by email and the names and other details relating to the winner/s may be published in a Australian Doctor Group publication. The gift cards will be mailed to winners’ addresses as stipulated in email correspondence.
Prizes will be awarded to the person named in the entry.
Prizes cannot be transferred, exchanged, or redeemed for cash.
The Promoter and its associates assume no responsibility for any error or delay related to the competition.
If the prize is unavailable, for whatever reason, the Promoter has the right to substitute the prize for a prize of equal or greater value, subject to State and Territory legislation.
The Promoter does not exclude any rights and remedies in respect of goods or services under the Australian Consumer Law in the Competition and Consumer Act (2010) (Australian Consumer Law) which cannot be excluded, restricted or modified. However, the remainder of this clause will apply to the fullest extent permitted by law and the Promoter will not be liable for any loss or damage whatsoever (including but not limited to indirect or consequential loss) or for any personal injury suffered or sustained in connection with any prize/s.
All entries become the property of the Promoter, (with the exception of any intellectual property rights comprised therein). As a condition of entering this competition you acknowledge that Australian Doctor Group may contact you via email with information that may be relevant to you and your industry.
