John, a 32-year-old software engineer, presents with a five-day history of worsening shortness of breath.

John reports that, five nights ago, he woke in the middle of the night gasping for breath and having a ‘coughing fit’, with an associated sensation that something was stuck in his throat.

He then managed to get back to sleep, and when he woke the following morning, the strange feeling in his throat had resolved.

Since then, he has noticed increasing shortness of breath when walking, making his bed and when talking for long periods of time.
