Amelia is a 42-year-old woman who presents with months of persistent right-sided ear discomfort. She describes a dull, deep ache in and around the ear with intermittent pressure and a blocked-ear sensation. There is no associated rhinorrhoea, otorrhoea, fever, vertigo or hearing loss.
Further history identifies several jaw-related features. The pain is worse on waking and increases with eating and prolonged talking. Amelia reports right-sided jaw tightness and fatigue and often develops a temporal headache by the end of the day. Her symptoms became more noticeable after a recent posterior dental restoration, during which her mouth was held open for an extended period.
