Oddly puffy shins and frothy urine

A rare trip to the GP is in order for this typically well university student with bilateral pitting oedema and frothy urine

Daniel is a 19-year-old university student who presents to his GP with a one-month history of bilateral pitting oedema to his mid-shins and three months of oddly frothy urine. 

Daniel denies preceding fever or infective symptoms, has not had chest pain or palpitations, denies light-headedness and does not have dysuria or haematuria. Systems review is otherwise unremarkable. 

Daniel has a past medical history of beta-thalassemia minor (baseline haemoglobin in the normal range of 120-140g/L). He takes no regular medications and has no known drug allergies. 

He does not smoke, occasionally drinks alcohol in social settings and is not sexually active.
