Caleb, a three-year-old boy, is brought in by his parents with a four-day history of intermittent fever and soft stool.

He has vomited twice over the preceding 12 hours and has also become reluctant to eat and drink.

Caleb was delivered vaginally at term after an uncomplicated pregnancy. He has been cruising along the 10th percentile for weight and height consistently since birth.

He was breastfed as an infant and has always been a fussy eater.
