A middle-aged man presents with a two-month history of abdominal pain. Has an earlier incident come back to bite?

Isaac, a 56-year-old warehouse stocktaker, presents with a two-month history of dull ache across his lower posterior chest.

He also reports acute episodes of severe back pain accompanied by abdominal pain.

He experiences abdominal pain 3-4 times a day, with each episode lasting between 10 and 60 minutes.

There is no history of associated diarrhoea, constipation, vomiting or urinary tract symptoms.
