    Fighting for breath: treating uncontrolled asthma in the primary care setting

    • Uncontrolled asthma is due to factors other than asthma itself, such as nonadherence to treatment or incorrect inhalation technique.
    • Fewer than half of patients taking inhaled asthma medications adhere to their prescribed regimens.
    • Diagnosis should be confirmed with objective testing such as spirometry and/or peak flow recordings.
    • Always counsel against over-reliance on SABA reliever treatment.
    • Ensure patient has an up-to-date written asthma action plan.
    • When escalating therapy in uncontrolled asthma, the addition of non-steroidal agents such as LABA is generally favoured over increasing ICS dosage.
    • In difficult-to-treat asthma with frequent symptoms or flare-ups despite ICS or ICS/LABA therapy, consider intensification with the addition of a LAMA.
