Culture of ‘dismissing red flags’ at Northern Beaches Hospital, report finds
Staff at Northern Beaches Hospital worked amid a culture of “dismissing red flags” when a patient’s condition was deteriorating, an inquiry says.
A report into the controversial Sydney public-private partnership hospital was published without announcement on NSW’s Clinical Excellence Commission website earlier in August.
The inquiry found junior staff were hesitant to raise concerns when a patient was deteriorating, that there were delays in triggering emergency protocols and inconsistent use of sepsis pathways.
“A recurring theme is the need to strengthen the culture of escalation,” the report said.
