GP practice error in recording a child’s weight may have resulted in his death 17 years later: UK coroner

Alex Reid died in 2021 from COVID-19 vaccine-induced immune thrombotic thrombocytopenia and cerebral venous sinus thrombosis.
It was just six weeks after his second dose of Vaxzevria.
Although the 28-year-old was not yet eligible for vaccination due to his age, the GP clinic had invited him to receive the vaccine after its computer system flagged that his BMI was 68.97.
The number was (obviously) wrong, but it meant he was classed a vulnerable patient who should be offered the vaccine.
