A hospital whose staff did not properly check on a woman before her suicide has been charged with breaching occupational health and safety legislation.

Shortly after giving birth to her daughter Lily in November 2023, Sarah Skillington experienced significant anxiety.

Five days later, on the recommendation of her doctor, she was admitted to Mitcham Private Hospital for additional support.

The 33-year-old was last seen alive by her husband at 10.27pm on 18 November. Her body was discovered 10 hours later.
