The cause of the “catastrophic” blunder that led to hundreds of UK registrars being wrongly told they had passed an exam they sat 18 months ago has been revealed.

Last week, the UK college of physicians informed 222 candidates they had actually failed the written exam in September 2023 and would need to resit it.

A further 61 registrars at the time were incorrectly told they had failed the make-or-break exam which determines progress into subspecialty training, such as cardiology, respiratory or intensive care.

The Federation of the Royal Colleges of Physicians said: “An error occurred when uploading the answer data from the digital exam provider to the [college] database and then aligning it with the answer key held by [the college].
