Surgery on wrong site and retained dressings among ‘record’ number of medical errors
The Victorian report says the higher number of adverse events reported reflects greater transparency.
Lack of recognition of clinical deterioration made up one third of all sentinel events and half of paediatric events over 12 months, a Victorian report has found.
The cases were among 245 sentinel or ‘harm events’ reported in Victorian hospitals in the 2022/2023 financial year, which included 167 deaths.
