GP Dr Chin Ven Tan was found guilty last year of altering medical records on an “industrial scale” to conceal inappropriate Medicare claims.

According to the NSW Civil and Administrative Tribunal, Dr Ven Tan updated clinical records retrospectively with fake referrals and reasons for patients’ chronic disease management plans.

But he was caught out.

The PSR ran 111 patient records through software known as “Parser”, which could detect when records were altered way after the consultation dates.
