My big problem with outcome-based funding

I recently read an article in which the author suggested health reform through a shift to outcome-based funding of general practice (as opposed to fee-for-service) using innovations in digital technology and data collection.
The immediate problem is, what exactly will be measured?
Looking back over my years in practice, I would assess 10 of my most significant outcomes to be all relationship-based and, therefore, unlikely to be measured by data collection:
The data collected in an outcomes-based funding program, I imagine, would be quantitative, not qualitative.
