The debate on the value and evidence base for pharmacist home medication reviews has erupted again.

Writing on the MJA Insight website, Dr Evan Ackermann focused on those programs being funded through the Community Pharmacy Agreements, including both the Home Medicines Reviews and the Residential Medication Management Reviews being conducted by accredited pharmacists.

For some, this debate may seem surprising. These reviews are initiated by GPs and are widely seen as key in the battle against the significant and widespread harm to patients resulting from medication misadventure.
