<p>Mia is a 21-year-old student living in residential accommodation who books an urgent telehealth appointment because her nose has suddenly changed colour.</p><p>Mia is typically well, has no past history of note and was vaccinated against meningococcal ACWY and B last year.</p><p>She reports that, in the past 24 hours, she has developed increasing nasal erythema, progressing today to non-blanching purple-blue discolouration.</p><p>There is no history of trauma. The nasal piercing is longstanding and not symptomatic.</p><p>Mia has had right lower abdominal pain and vomiting for 24 hours and is feeling progressively light-headed.</p><p>She denies any headache, photophobia, neck stiffness or fever but is apparently unwell, appearing clammy, lethargic and pale, in addition to the nasal finding (pictured).</p>
