<p>Jamie is a 23-year-old account manager who presents for review of enlarging, tender scrotal and thigh ulcers present for the past week.</p><p>Two weeks prior, Jamie had flu-like symptoms that resolved without treatment.</p><p>Jamie is in a long-term monogamous heterosexual relationship, with no other sexual contacts. Both Jamie and his partner have previously screened negative for STIs.</p><p>Jamie is generally well, takes no regular medications and has no past history of note. On examination, vital signs are normal.</p><p>There are five tender oval ulcers between 0.5 and 4cm in diameter, affecting the left side of the scrotum and the adjacent surface of the left upper thigh (pictured).</p><p>There is no inguinal lymphadenopathy or oral ulcers. Swabs of the ulcers are negative for HSV, syphilis and mpox, as are HIV, hepatitis B and C, and syphilis serologies.</p><p>EBV IgM is positive and IgG is negative. There is leucocytosis and elevated CRP with otherwise normal FBC and EUC.</p>
