<p>Mike is a 44-year-old otherwise healthy male who presents with two days of fever, malaise and a spreading rash.</p><p>Three days prior to the onset of these symptoms, Mike had been prescribed flucloxacillin to treat a furuncle on his left thigh, which was improving.</p><p>The rash is hot, swollen and itchy, affecting the face, trunk and proximal limbs symmetrically.</p><p>Mike is usually well, with no past or family history of note and no known allergies. He had chickenpox as a child.</p><p>On examination, Mike is febrile at 38.3°C, with a mild tachycardia and normal blood pressure.</p><p>He has a generalised coalescing erythematous maculopapular rash with multiple small, non-follicular pustules in the erythematous areas.</p><p>The rash is particularly marked in the axillae and groin creases.</p>
