What is causing this infant’s pruritus?

<p>Dylan is a 12-month-old with a one-month history of a spreading itchy rash that developed a few weeks after a camping holiday with extended family and friends.</p><p>Dylan is systemically well, thriving, with no past history of note. His mother and older brother have eczema, and both have also been experiencing a flare.</p><p>Dylan’s mum has been treating all three with OTC hydrocortisone 1%, with no effect.</p><p>On examination, Dylan has a polymorphic eruption with erythematous papules and coalescing plaques (pictured).</p><p>There are crusted papules affecting the metacarpophalangeal joints, soles of the feet and left periorbital region. Examination is otherwise unremarkable.</p>
