The long-term nature of chronic disease requires a shift in approach by GPs, with the emphasis moving from the immediate concerns of the person on the day, to a longer-term partnership that addresses the challenges of the condition over time.
By necessity, this relationship will encompass the inextricably intertwined physical and mental wellbeing of the person.
