Hospital's failure to contact GP criticised after patient's suicide


A GP was left in the dark about a suicidal patient because the hospital did not send a discharge summary until the day after she killed herself, a coroner has found.

Naomi Watson-Ley, 33, whose history included two suicide attempts and ECT for major depression, spent two nights at a psychiatric unit in Royal North Shore Hospital after expressing further suicidal thoughts in June 2015.
