Pharmacy mistakenly dispenses digoxin instead of diuretic

AN INEXPERIENCED Canberra pharmacist has been reprimanded for dispensing a potentially fatal dosage of digoxin to a patient with chronic kidney failure which caused his dialysis to be brought forward.

Mandy Wang was the overseeing pharmacist on duty at The Chemist Warehouse in Phillip, ACT, in May 2012 when a dispensary assistant accidentally supplied the wrong medication, digoxin at a dosage of 250mcg twice daily instead of 250mg twice daily of the diuretic Lasix.
